How to Submit Your Child’s Vaccine Records in Ontario

What is a Child’s Vaccine Record?
The “Vaccine Record Card” (also known as the Personal Immunization Yellow Card) is a physical booklet or card where a child’s doctor or healthcare provider records their vaccination history. It serves as proof of immunization.
Immunization of School Pupils Act, 1990 (ISPA) and the Child Care and Early Years Act, 2014 (CCEYA) require that students be vaccinated against nine designated diseases or have a valid exemption. Those vaccines are; Measles, Mumps, Rubella, Diphtheria, Tetanus, Polio, Meningococcal, Pertussis (whooping cough), Varicella (chickenpox) – only applies to students born on or after 2010.
Parents or guardians must use the information on the yellow card to report vaccinations to Toronto Public Health (or respective local public health unit), either by entering the details online or by submitting a copy via email, fax, or mail.
How to Submit Your Child’s Immunization Record to Toronto Public Health
In Toronto, you should submit your child’s yellow immunization card directly to Toronto Public Health (TPH), not to the school. Parents or guardians must report their child’s vaccination records to the local public health unit. Teens who are 16 or 17 years old can report their own records. Schools do not collect or forward these records on your behalf. Reporting options as follows.
- Online via ICON (Immunization Connect Ontario):
- Visit toronto.ca/Student Vaccines to access the ICON system.
- Log in using your child’s Ontario Health Card Number or Ontario Immunization ID (OIID).
- If you need assistance obtaining an OIID or PIN, call TPH at 416-338-7600 and select option 2.
- By Mail:
Send a copy of the immunization record to:
Toronto Public Health
Vaccine Preventable Diseases
5100 Yonge Street, 2nd Floor, Toronto, ON, M2N 5V7
- By Fax:
Fax the record to 416-338-2487. Include your child’s name, date of birth, OIID or health card number, and your contact information on the cover sheet

What is the ICON System?
The ICON (Immunization Connect Ontario) system is a secure, online tool where parents or guardians can report and update their child’s immunization records directly to their local public health unit. It is easy to use and helps ensure your child meets Ontario’s school vaccine requirements.

If you are New to Ontario: How to Submit your international records
If you have recently moved to Ontario from another province or country, you still need to submit your child’s immunization records to Toronto Public Health (or your local public health unit) before attend school.
Bring any records from your previous province or country. These may be a yellow card, vaccination booklet, hospital or international record. Documents in another language are accepted. Translations are helpful but not mandatory.
Take your child to a family doctor or a walk-in clinic in Ontario. The doctor can:
- Review the existing immunizations
- Give any missing vaccines to meet Ontario’s school requirements
- Fill out the Ontario immunization card, if needed
Then you need to submit the records through the ICON system, by email, fax, or mail.

What should parents do if their child receives an immunization-warning letter?
If your child receives an immunization-warning letter from Toronto Public Health, do not panic. This letter means their vaccination record is incomplete or missing required information. Start by reviewing your child’s immunization records to check if any vaccines are missing or not yet reported.
If the records have not been submitted yet: If your child has received all required vaccines, submit the updated record to Toronto Public Health immediately through the ICON system, by email, fax, or mail.
If any vaccines are missing: Book a catch-up appointment with your healthcare provider as soon as possible or make an appointment at a community clinic online toronto.ca/Student Vaccines to get vaccinated for free.

What to Do if Your Child Doesn’t Have OHIP?
Required vaccines for school are publicly funded and free, even without OHIP. No child will be denied school-required vaccines due to lack of health coverage.
Toronto Public Health offers immunization clinics for children without OHIP. You can also contact a Community Health Centre (CHC), which often provides care to uninsured families. Bring any vaccine documents from your home country or previous province. A nurse will review them and advise what is needed.
Call Toronto Public Health at 416-338-7600 to ask where you can get vaccines without OHIP. Ask for interpreter services if needed.
