Acquired Brain Injury (ABI)

About this Program

The ABI Outreach Team is a new community-based service for people that are living with an acquired brain injury (ABI) in Scarborough. This multidisciplinary team of professionals will provide support to people with an ABI and their families in order to help them learn new skills, access community resources, and optimize their quality of life. As a partnership between Scarborough Centre for Healthy Communities and Cota, the new service is composed of staff from both organizations, working cohesively as one team to support the healthcare needs of the clients.

​The team is composed of the following professionals:

  • A Nurse,
  • An Occupational Therapist,
  • A Behaviour Therapist,
  • A Case Manager
  • A part-time Psychologist,
  • ​Workers/Individual Support Workers

Eligibility

In order to be eligible for the new service, applicants must:

  • Have an acquired brain injury
  • Live in Scarborough
  • Be between the age of 16 and 64 years old
  • ​Need support in the community as a result of an acquired brain injury
