Welcome to Social Prescribing! 

We are thrilled to introduce a new initiative to enhance the health and well-being of our community members in Scarborough through the innovative practice of social prescribing.  

At SCHC, we believe that good health extends beyond medical treatments and interventions. We recognize the importance of addressing social determinants of health and offering holistic support to individuals seeking to improve their well-being.  

What is Social Prescribing?

Social prescribing is a way to support your overall well-being by connecting you with activities and programs in your community that can improve your health and happiness. It recognizes that good health goes beyond medical treatments and considers other factors influencing your well-being, such as social connections, physical activity, creativity, and learning.  

Through social prescribing, we aim to empower our clients to take an active role in their health journey by connecting them with the resources, support, and opportunities available in the community that can make a positive difference in their lives. 

How to get involved:

If you are interested in participating in our Social Prescribing Program at SCHC, there are several ways to get started. You can ask your healthcare provider for a referral or reach out to our team directly to learn more about the program and how it can benefit you. We are here to guide you through the process and connect you with the appropriate resources and support.
