About this Program
The Lawrence East Partnership Program (LEPP) is a collaborative community-based program. Using an interdisciplinary model of care, LEPP offers residents holistic support and services.
Scarborough Centre for Healthy Communities has partnered with Fred Victor, Cota and Toronto Community Housing to support our community with the following team:
- Registered Nurse
- Housing Coordinator
- Independent Support Worker
- Mental Health Case Managers
- Peer Support Worker
- Outreach Coordinator
- Community Outreach Worker
The main LEPP offices are located on the first floor of 4175 Lawrence Ave East in apartments 1A and 1B. Please drop by to learn more about our program or contact us through our Program Administration.
LEPP services are available to residents of 4175 and 4205 Lawrence Ave East; however, some short-term supports are also available to the surrounding community. LEPP also welcomes Interprofessional Practice Care Team members to provide services to residents. Ask LEPP staff about accessing these services.

LEPP Outreach Team
LEPP team members are passionate about engaging with our residents and helping their voices be heard. We invite 4175 and 4205 Lawrence residents to join the Resident Engagement Advisory Committee (REAC). Residents involved in REAC engage their neighbours in discussions about programming, sharing resource information, building personal capacity and planning events for the community.
