SCHC, together with local health partners and family physicians, is working to expand access to primary care for all Scarborough residents.
If you do not have access to a primary care provider – such as a family physician or nurse practitioner – and are looking for one, please complete the registration form below. Our team will review your information and connect you with a provider, either at SCHC or with another physician in the Scarborough community. You will hear from us by phone or email.
Please note: matching you with the right provider may take time. It may take up to 3 months before you hear back from us. We appreciate your patience as we work to find the best possible fit for your care.
If you need urgent care, you can visit the Scarborough Urgent Care Centre (Walk-in).
If you are uninsured, pregnant, have recently given birth, or have a child under four years old, we ask that you call us at 416-642-9445 to complete the registration process.
Please note that it may take 10 minutes to complete this form as it will ask you about your current health status and medical history.
A separate form is required to be completed for each individual seeking care including children, spouses and other family members.
