About this Program
Scarborough Centre for Healthy Communities (SCHC) offers comprehensive primary health care and interprofessional care services at multiple sites across Scarborough. We provide direct primary care with a doctor or nurse practitioner as well as coordinated care and access to our team of interprofessional health providers consisting of registered nurses, social workers, dietitians, physiotherapists, chiropodists (foot care specialists), mental health case workers, harm reduction workers, and medical secretaries.
We provide care for clients experiencing a one-time episode of illness or injury (e.g., sore throat, sprained ankle) and clients who require ongoing support for chronic conditions (e.g., diabetes, asthma, high blood pressure).
Our client-centered approach ensures that clients are actively involved in planning and decision-making about their care. We provide culturally safe services and interpretation upon request.
We see clients with or without a health card, including OHIP, Interim Federal Health (IFH), and clients with no status.
PLEASE NOTE: To ensure timely care, we encourage clients to call ahead to book an appointment. Evening hours are available.

Services
Medical Services:
- Periodic health exams, well-baby visits, sexual health screenings
- Prenatal care
- Chronic disease management (e.g., hypertension, diabetes)
- Immunizations for children and adults
- Women’s health, LGBTQ2+ care, HIV Pre-Exposure Prophylaxis (PrEP), and medical transition care
Counselling and Support:
- Mental health counselling for depression, anxiety, addictions, trauma, family conflict, grief, and more
- Social support services
- Community workshops, groups, and presentations
Specialized Services:
- Chiropody / Foot Care (for seniors, adults, children, and clients with underlying conditions)
- Physiotherapy (doctor’s referral required)
- Nutrition counselling
- Harm reduction services (clean supplies, needle exchange, support for substance use)
- Smoking cessation
- Offloading foot care devices: Medical products designed to relieve pressure on the foot, reduce repetitive trauma, and protect wound sites to promote healing
Child and Family Services:
- Care4Kids: Wraparound care for children 0–5 years old without a primary care provider, supported by nurse practitioners, social workers, and RPNs
Eligibility
- Clients living or working in Scarborough and surrounding areas
- Accepting referrals from family doctors, nurse practitioners, or self-referrals (for most programs)
- Services are not available to visitors, students, or super visa holders
- Certain services (e.g., physiotherapy) require a doctor’s referral
