About this Program
Hospice palliative care is a philosophy of care for individuals living with or dying from advanced illness and those who are bereaved. Hospice palliative care neither hastens death nor prolongs life.
Hospice palliative care is intended to provide the best quality of life by managing pain, distress, and symptoms related to the physical, emotional, spiritual, psychological, social and cultural needs of the person and their family.
The role of the Palliative Pain and Symptom Management Consultant (PPSMC) is to provide palliative care education to health care providers through education sessions, consultation, coaching and mentoring. The PPSMC advances expertise in delivering palliative care for all disciplines across the continuum of care, for all life-limiting diagnoses and all ages, from pediatric to elderly.
PPSMC clients are health care providers. The PPSMC does not provide direct care. The PPSMC role and program is not an emergency service.
The PPSMC service does not include responding to inquiries or education activities for the general public.
The PPSMC supports capacity building of front line health care providers in palliative care, pain and symptom management supporting a palliative care approach in a variety of care settings:
Long Term Care Home sector:
- Central East LHIN
- Central East LHIN Home and Community Care Contract Providers
- Community Support Service Providers
- Primary Care (Physicians and Nurse Practitioners, community based resource teams
The PPSMC supports health care providers through:
- Consultation – in person, by telephone, by email, videoconference
- Education – customized for the individual and/or organization
- Coaching and Mentorship
- Linkages to palliative care resources
