Community Health Centre
About this Program
We have three community health centres (CHCs) located across Scarborough. Our CHCs are staffed by doctors, nurse practitioners, registered nurses, social workers, dietitians, chiropodists (foot care specialists), and medical secretaries. Together, they provide medical care and health-related services such as check-ups, immunizations, diabetic foot care, nutrition counselling, social support services, and harm reduction (free condom program, needle exchange, point of care HIV testing). The team sees clients who experience a one-time episode of illness or injury (ex: having a sore throat, spraining an ankle) and clients who require ongoing care such as treatment for chronic health conditions (ex: diabetes, asthma, high blood pressure).

At SCHC, we believe in a client-centered approach to primary health care. This means that our interdisciplinary team of healthcare professionals works with clients to ensure they are actively involved in the planning and decision-making about their care.
The team will see clients with or without a health card to remove barriers to accessing high-quality health care. We understand the importance of culturally sensitive care and work to ensure our clients’ preferences are respected.
If you need a translation service, please let us know ahead of time, and we will provide it free of charge.
We are not a walk-in clinic; thus, all visits require booking an appointment. Our goal is to be a permanent healthcare “hub” for our clients so that they feel less need to use walk-in clinics or local emergency departments. As a Community Health Centre, all services provided are free of cost. We serve clients with OHIP, Interim Federal Health (IFH), and no status.
Primary Health Care involves the following:
- Managing chronic health conditions such as hypertension, diabetes, etc.
- Health Promotion (Preventing clients from becoming ill or injured)
- Treating acute (sudden onset) or episodic illness
- Referrals to/coordination with other health care professionals (such as hospitals and specialist care)
- Immunizations for children and adults
- Prenatal Care
- Counselling and Support Services
Services
Medical Services: Our team of Doctors, Nurse Practitioners, and Nurses work together to provide you with the best possible care. Services include periodic health exams, well-baby visits, sexual health screening services, and smoking cessation.
Chiropody (Foot Care): Our registered chiropodist provides comprehensive foot and leg care services. These services are offered to seniors, people with disabilities, adults, and children with underlying medical conditions such as diabetes, dermatological problems, or other foot care needs.
Counselling: Our counsellors assist clients, families and the community by strengthening coping skills and finding effective solutions to individual sources of stress or problems. Our counsellors help clients address mental health issues that include depression, anxiety, addictions, current and/or historical abuse/trauma, family conflict, adjustment, loss, grief, and more.
Care4Kids: Wraparound care for children 0-5 years old who do not have a primary care provider and their immediate families. Children will be supported by a Nurse Practitioner, Social Worker and/or Registered Practical Nurse.
